TERMS AND CONDITIONS

I hereby assign full copyright of these photographs (hereafter referred to as "The Material") to JPHii DESIGN (and the related representatives and assigns) together with the right of reproduction either wholly or in part. I grant to JPHii DESIGN or licensees or assignees the permission to use The Material either separately or together, either wholly or in part, the perpetual and irrevocable and unrestricted right to use and publish video and/or photographs of me, taken by me, or featuring my work, or where I may be included for editorial trade, product advertising and such other fashion /business purpose in any manner and medium. JPHii DESIGN and licensees or assignees may have unrestricted use of these for whatever purpose, including advertising, with any retouching or alteration without restriction. I agree that The Material and any reproductions shall be deemed to represent an imaginary person, and further agree that JPHii DESIGN or any person authorized by or acting on their behalf may use the above mentioned photographs or any reproductions of them for any advertising purposes or for the purpose of illustrating any wording, and agree that no such wording shall be considered to be attributed to me personally unless my name is used. Provided my name is not mentioned in connection with any other statement or wording which may be attributed to me personally, I undertake not to Prosecute or to institute proceedings, claims or demands against either JPHii DESIGN or their agents in respect of any usage of The Material. I hereby release JPHii DESIGN from all claims and liability relating to images, video or photographs taken of me. I agree that if any issues or concerns may arise in the use of The Material, or during the production of The Material, that I will address these issues and concerns directly with JPHii DESIGN, and will not do so publicly or in any way that will tarnish the reputation of JPHii DESIGN. I agree that JPHii DESIGN will provide material payment for my participation in the production of The Material, through either financial or material form. This payment will be agreed upon prior to the production of the The Material. I have read this release form carefully and fully understand its meanings and implications.
